ICanQuit+ Mobile Application Privacy Statement
Promote-UP (We or Us), a research study of the iCanQuit+ application for smoking cessation at the University of Florida, Gainesville, Florida, takes your privacy seriously. We respectfully request that you review our Privacy Statement carefully so you understand our privacy practices. This Privacy Statement explains how We collect, use, disclose, and safeguard the information you provide to Us so you can make an informed decision when using ICanQuit+. This Privacy Statement covers both our website and mobile application (Service) solely for the clinical trial which requires you to use iCanQuit+. By using iCanQuit+ or otherwise providing your information to Us, you agree to the terms of this Privacy Statement. If you do not agree with the terms of this Privacy Statement, please stop using iCanQuit+.
INFORMATION WE COLLECT
We collect some non-personal information and some personally identifiable information (PII) when you access ICanQuit+ through the mobile application. Non-personal information includes information that cannot be used to identify you, like anonymous usage data, the number of clicks you make as you navigate the website or mobile application, etc. Personal Information that may be collected includes information that can be used to identify you such as your first name, email address, and photograph. The University of Florida’s Human Subjects Institutional Review Board regulates the collection of this information.
HOW WE USE PERSONAL INFORMATION
When using ICanQuit+, We may ask you to provide Us with some personally identifiable information to create your User profile and to support your use of ICanQuit+. We describe how We use or disclose your personal information in more detail here:
User Information. ICanQuit+ includes an engaging and personalized experience for quit smoking program participants (Users). To effectively deliver our ICanQuit+ to our Users We may collect, store, and use personally identifiable information that you provided to Us when you registered for ICanQuit+. The use of this information is regulated by the University of Florida’s Human Subjects Institutional Review Board.
Communications. We may use personally identifiable information you provided to Us to respond to you, to provide what you requested, to update you about changes to our Privacy Statement or terms and conditions.
Research. We use identified information (information that is linked to your study identification number) to measure ICanQuit+’s impact and effectiveness to Users, and to conduct surveys and continue our research at the Fred Hutchinson Cancer Research Center, which may result in published reports or articles. De-identified information is used in our reports, research, surveys and results. In order to protect your privacy, We do not include any personally identifiable information in our research reports.
HOW WE PROTECT YOUR PERSONAL INFORMATION
When you share personally identifiable information with Us it is protected both online and offline. When We collect sensitive information like your name and contact information, the information is encrypted and transmitted in a secure manner. This means that your information is encrypted when at rest and in transit. The computers and servers We use to store this information are kept in secure environments. Only employees of the University of Florida or the Fred Hutchinson Cancer Research Center with the need to access your information for the research study are granted access to your personally identifiable information.
ACCESS AND CONTROL TO YOUR PERSONAL IDENTIFIABLE INFORMATION
Your Rights. We are required by law to protect the privacy of your Personal Information, provide you this notice explaining our responsibilities and privacy practices when We collect your Personal Information, notify you of a breach of unsecured Personal Information, and follow the terms of our Notice.
Data storage. We store Personal Information and other data for as long as it is allowed by the University of Florida’s Institutional Review Board.
Study participants can delete the app from their phones at any time. However, this will not automatically delete or disable their accounts or the associated data that has already been collected. You can request to have your Personal Information deleted by contacting the study team.
MISCELLANEOUS PROVISIONS
Devices. We may collect information from or about the computers, phones, or other devices where you install or access ICanQuit+, depending on the permissions you've granted. We may associate the information We collect from your different devices, which helps Us provide consistent use of ICanQuit+ across your devices. For example, connection information such as the name of your mobile operator or ISP, browser type, language and time zone, and IP address. The app does not collect or store location data.
Cookies. We may use cookies and similar technologies to provide support of ICanQuit+.
Legal Proceedings. We may disclose your Personal Information in response to a court or administrative order, subpoena, discovery request, or other lawful processes.
UPDATES TO PRIVACY STATEMENT
We will notify you through our Website or Apps of any significant changes made to our privacy practices by updating our Privacy Statement, including the opportunity to stop using ICanQuit+.
